Salary:
• Between $26.38 and $35.22 per hour, depending on experience.

Key Responsibilities –Community Worker - Remote (Montreal).
Provide regular follow-up for clients placed in the south in long-term care facilities (CHSLD), group homes, and foster families monitored by northern social services.
• Offer general services to clients of the DI-TSA and PLA programs in collaboration with other members of the CLSC Social Services team.
• Assist in completing and re-evaluating autonomy assessments using the multi-clientele grid in collaboration with the PLA team (nurses, community workers, rehabilitation technicians, etc.). The role involves translating and explaining documents to clients in Inuktitut.
• Contribute to creating and maintaining intervention plans for all files and ISP (Individual Service Plans).
• Coordinate and assist in organizing visits for northern family members to clients placed in the south.
• Provide help, advice, and support to clients and their families in collaboration with CLSC Social Services members.
• Offer assistance, advice, and support to caregivers.
• Participate in evaluations and residential placement requests, as needed.
• Contribute to developing services for seniors and other individuals with reduced autonomy, if necessary.
• Plan and deliver prevention and promotional activities with partners (PLA team, Social Services, Community Wellness TC, community, etc.).
• Fulfill clinical record-keeping and weekly statistical reporting requirements.
• Prepare and lead public awareness and prevention activities.

Education Requirements:
• High school diploma or equivalent experience.
• If the candidate has a college diploma in social techniques, the job title will be adjusted accordingly.
Experience Requirements:
• Previous experience in a role involving significant interpersonal interactions is an asset.
Other Requirements:
• Must reside in the Montreal region, suburbs, or South Shore.
• Valid driver’s license.
• Bilingual, with Inuktitut being one of the two required languages.
• Flexibility and openness.
• Willingness to explore diverse perspectives and approaches to individual, family, and social issues.
• Adaptability, especially regarding intervention techniques and strategies.
• Recognition and respect for Inuit culture and values.
